About The Clinic
At Prime Care Family Health Team (PCFHT), we believe all people have the right to access safe, respectful, and affirming healthcare that acknowledges their gender identity. We firmly denounce discrimination targeting people who are transgender, non-binary, or gender diverse. We are dedicated to providing safe, accessible spaces and gender affirming care within a multi disciplined shared care model. We welcome collaboration with like-minded healthcare organizations, healthcare providers, the 2SLGBTQ+ community, and community organizations to ensure folks can access the care they need and live their lives authentically. At PCFHT, we understand that gender diverse folks are experts in their own care needs. We look forward to working with you to achieve your goals.
What We Offer
PCFHT provides a range of gender-affirming healthcare and counseling services to individuals ages 16+, including:
- Education and information on gender identity and healthcare options
- Support for social, medical, and legal transition processes
- Surgical assessments, referrals, and support
- Sexual health education and screening
- Individualized support for hormone therapy and monitoring
- Psychosocial support throughout the transition process
- Group therapy sessions and workshops
Referral Process
We aim to eliminate barriers to accessing care:
- Individuals can self-refer to PCFHT Gender-affirming care services.
- Referrals can also be made by any healthcare provider, friend, family member, caregiver, or school counselor with the individual’s consent.
What to Expect During Your Visit
The first visit is about getting to know you, learning your expectations and goals, and developing an initial understanding of your well-being and gender identity. You will also have a chance to learn more about our services and the various team members at the clinic. It is important to know that medications may not be prescribed at this initial visit. It takes time for us to get to know you and cover all the information necessary for you to decide on future steps.
You are welcome to bring any support person you wish to your appointments.
