A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you – the patient.
The Family Health Care Team is designed to give doctors support from other complementary professionals. Most Family Health Teams will consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills, so that you receive the very best care, when you need it, as close to home as possible. All programs and services are available at no charge to rostered patients of the Prime Care Family Health Team.
We offer weekday evenings and Saturday Clinic hours. See the After Hours Care section for Clinic hours by clicking on the link below:
Monday – Thursday: 6pm to 8pm
Saturday: 9am to 12pm
Fridays, Sundays, and Holidays: Closed
The closest lab is Life Labs which is conveniently located in our building.
You will need to present your Health Card at the Reception Counter with each visit. This will ensure that your Health Card information is up-to-date in our system. Please advise us if there has been a change in address and/or telephone number as well.
Yes. Our Family Health Team uses an EMR system (Electronic Medical Records), and as a result, all information in our system can be viewed by your physician.
